SD Rehab accepts most insurance plans when evaluation and treatment is prescribed by a physician. Claims are filed directly by SD Rehab to facilitate and expedite insurance reimbursement, however reimbursement for services is based on individual policies. SD Rehab is a preferred provider of Blue Cross/Blue Shield of Illinois and a participating member of Private Health Care Systems (PHCS).
